Scenario Hook

After the party accidentally murdered the quest-giving NPC and sold his keep to a dragon, the region has fallen into chaos. A mysterious innkeeper named Maren Ashveilsummons the party to the Last Inn — a tavern that only appears to adventurers who've truly screwed up. She offers them one chance to set things right: retrieve the Quill of Undoing from the Shattered Vault beneath the inn before dawn, or the timeline collapses for good.

Encounter: The Guilt Wraiths

In the Shattered Vault's first chamber, the party faces 3 Guilt Wraiths — spectral echoes of the consequences they caused. Each wraith takes the form of an NPC the party wronged. They attack with psychic damage and force Wisdom saves (DC 14) or the target is frightened for 1 round.
